Nurse Practitioner - PRN/Part Time ~ Fully Remote

Role Overview

The Nurse Practitioner (VSP) applies advanced education and clinical competencies to achieve optimal patient outcomes on a short-term 6-month basis, focusing on osteoporosis management for women.

Responsibilities

• Calling Humana members to discuss open care gaps and placing needed orders, specifically for women who had a fracture in the last 6 months.
• Creating and registering patient records within Athena.
• Verifying benefits, gathering medical and surgical history, and verifying and entering medications.
• Coordinating management of members' social needs to facilitate gap closure and providing transportation services.
• Connecting with outside partners to ensure testing is completed prior to member-specific timelines.
• Ensuring adequate follow-up including receipt of results, medication initiation, and PCP follow-up.

Qualifications

• Bachelor's degree and Master of Science in Nursing (NP).
• Active, unrestricted NP license in required states.
• Excellent verbal and written communication skills.
• High level of skill in interpersonal relationships and team-based care.
• Knowledge of HEDIS measures and quality indicators preferred.
• Experience managing Medicare Advantage panels preferred.
