Role Overview
Walker Healthforce is seeking an RN Case Manager with 3 years of experience for a 12-month contract opportunity in Michigan.
Core Requirements
- 3 years of clinical nursing experience in a clinical, acute/post-acute care, or community setting
- 1 year of case management experience in a managed care setting
- Experience managing patients telephonically and via digital channels
- MS Office computer skills (Word and Excel)
- Excellent interpersonal and communication skills
- Current, active, and unrestricted Michigan Registered Nurse (RN) license
- Nursing Diploma or Associate's degree in nursing
Preferred Qualifications
- Bachelor’s degree in nursing (BSN)
- Certification in Case Management (CCM)
- Certification in Chronic Care Professional (CCP)
Job Responsibilities
- Coordinate a multidisciplinary team to deliver holistic, person-centric care management
- Serve as the single point of contact for members, caregivers, and providers via phone, email, and text
- Assess, develop, implement, monitor, and evaluate care plans to optimize member health
