Remote Telehealth Nurse Practitioner Per diem weekends

Role Overview

The Advanced Practice Provider (APP), CareLine, PRN is responsible for diagnostic patient care primarily through virtual, remote consultation via video conference or telephone. Serving as the dispositional authority for after-hours and weekend clinical calls, this role assesses, diagnoses, treats, and documents geriatric patients' health status.

Essential Job Duties & Responsibilities

• Provide diagnostic patient care via virtual video conference or telephone.
• Assess acute and non-acute clinical problems, perform physical evaluations, and develop patient management plans.
• Evaluate the need for immediate nursing intervention, consultation, and/or referral.
• Confer with primary care physicians and medical providers to report health data and ensure compliance.
• Document assessments, interventions, and progress toward outcomes accurately.

Requirements & Qualifications

• Bachelor's degree in Nursing (BSN) and graduate of an Advanced Practice Nursing program; Master's degree in Nursing required.
• Minimum of 3 years of acute or primary care clinical work experience.
• Multi-state licensure including FL, VA, and at least two additional states.
• Valid, active Registered Nurse and Nurse Practitioner board certification (AANP or ANCC).
