Role Overview
The Advanced Practice Provider (APP) I, CareLine is responsible for diagnostic patient care primarily through virtual, remote consultation via video conference or telephone. Serves as the dispositional authority for after-hours and weekend clinical calls.
Schedule
- Friday meeting: 1530-1630
- Saturdays: 0900-2030
- Sundays: 0900-2030
Essential Job Duties
- Assess acute and non-acute clinical problems via virtual video conference or telephone.
- Perform and document physical evaluations and patient histories, analyzing trends in patient conditions.
- Develop, document, and implement patient management plans including health education, physician referrals, and case management.
- Confer with patient PCPs and medical providers to report health data and ensure compliance.
- Collaborate with on-call PCPs to support clinical outcomes using telemedicine.
- Participate in clinical rounds, conferences, risk and quality management programs.
Knowledge, Skills, and Abilities
- Travel requirements for onsite observation and medical record review depending on state laws (e.g., Georgia quarterly, Tennessee bi-annually).
- Expert knowledge of medical practices to function independently.
- Technical capability to conduct telemedicine visits.
- Proficiency in Microsoft Office Suite (Excel, Word, PowerPoint, Outlook).
- Bilingual (Spanish/Creole) is a plus.
