Job Summary
We are seeking experienced Registered Nurses (RN) with strong Managed Care and DSNP (Dual Special Needs Plan) experience to join a remote Case Management team.
Key Responsibilities
- Develop and manage individualized care plans (ICP)
- Complete health risk assessments and care management activities
- Coordinate transitions of care and discharge planning
- Evaluate member treatment plans and healthcare needs
- Collaborate with providers, care teams, and community resources
- Identify barriers to care and implement solutions
- Refer members to Disease Management, Quality Management, Mental Health, and Social Services
- Ensure compliance with clinical guidelines and regulatory requirements
- Monitor member progress and update care plans as needed
Required Qualifications
- Active Registered Nurse (RN) License
- Managed Care experience, specifically DSNP
- Strong Microsoft Office proficiency
- Excellent documentation and organizational skills
- Experience using multiple medical record systems
Preferred Qualifications
- Transitions of Care experience
- Case Management experience (3+ years)
- BSN Degree
