Job Description
Optum Home & Community Care is seeking a National On Call advanced practice clinician to provide telephonic and telehealth care, direction, and coordination for patients in various settings.
Scheduling & Availability
- Per diem, work-from-home position providing 24/7 coverage.
- Minimum requirement of 4 weekend shifts per month (32 hours in a four-week period).
- Additional availability of a minimum of 12 hours per month.
- Holiday coverage is required (including summer and winter holidays).
Primary Responsibilities
- Provide virtual care and utilize EMR proficiently to manage acute, chronic, medical, and behavioral conditions.
- Perform comprehensive assessments and document findings compliant with CMS regulations.
- Coordinate care across transitions and facilitate advanced care plan conversations.
- Participate in quality initiatives and complete mandatory training.
Qualifications
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- Active and unrestricted RN and NP license in the state of residence and Florida.
- Active Nurse Practitioner certification (FNP, ANP, GNP, or AG-ACNP).
- Current DEA licensure or ability to obtain.
