Role Overview
Optum Home & Community Care is seeking a full-time remote Telephonic Nurse Practitioner to provide virtual urgent and after-hours care.
Schedule & Availability
- 24/7 coverage model including evenings, nights, rotating weekends, and holidays.
- Minimum commitment of 20 hours per month.
- Every other weekend coverage required (day and night shifts).
- Holiday coverage rotation required.
Primary Responsibilities
- Deliver cost-effective, quality telephonic and telehealth care.
- Manage medical, behavioral, chronic, and acute conditions.
- Perform comprehensive assessments and document in compliance with CMS regulations.
- Coordinate care across transitions and communicate effectively with interdisciplinary teams.
