NP or PA, HouseCalls - Marion and Polk Counties, OR

About the Position

Optum Home & Community Care is seeking a Nurse Practitioner or Physician Assistant for the HouseCalls program in Marion and Polk Counties, OR. This non-prescribing, field-based role involves conducting annual in-home health assessments for Medicare Advantage members to improve health outcomes through education, gap closure, and collaboration with primary care providers.

Responsibilities

• Conduct comprehensive in-home assessments, including past medical history reviews, medication reconciliation, vital signs, and physical exams
• Perform evidence-based screenings and point-of-care testing
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to members' primary care providers to address gaps in care
• Recognize urgent or emergent situations and intervene appropriately
• Educate members on disease processes, medications, and compliance
• Address social determinants of health and provide referrals as needed

Requirements

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care for NPs, or current NCCPA certification and state licensure for PAs
• Active, unrestricted licensure in the state of practice
• Active and unrestricted driver's license and access to reliable transportation
• Ability to navigate varied home environments

Nice-to-Haves

• 1+ years of clinical experience in family, geriatric, or home health
• Proficiency with electronic medical records and technology
• Ability to transport equipment weighing up to 30 pounds and navigate stairs
• Proven communication skills with geriatric or Medicare populations
