Nurse Practitioner/PA (Remote 7AM - 7PM, Fri/Sat/Sun)

We are seeking a dynamic and compassionate Nurse Practitioner or Physician Assistant to join our multidisciplinary healthcare team in this fully remote position. In this pivotal role, you will provide high-quality, patient-centered care for multiple subacute rehab and skilled nursing facilities.

Schedule

• Day shift: 7:00 AM - 7:00 PM
• Frequency: 3 shifts x 12 hours (Thursday or Friday, Saturday, and Sunday)

Responsibilities

• Cover our on-call provider line for multiple subacute rehab and skilled nursing facilities across multiple states.
• Review labs, radiology reports, medications, consults, and pharmacy recommendations.
• Prescribe medications including narcotics and controlled substances when needed.
• Prescribe and order a variety of IV medications and IV fluids.
• Order diagnostic testing when needed (labs, x-rays, fluids, UA, etc.).
• Send patients to the ED or refer to specialists when needed.
• Perform other remote work as delegated by the Director of Remote Provider Services.

Requirements

• Valid state licensure as a Nurse Practitioner or Physician Assistant in New York, Pennsylvania, and Massachusetts.
• Current and valid New York DEA license.
• Proven experience in subacute rehab and skilled nursing facilities.
• Strong familiarity with EMR/EHR systems.
