RN- Complex Nurse Case Manager- Remote
The Cigna Group is hiring a remote Complex Nurse Case Manager to coordinate, support, and guide care for customers and their families. Candidates will utilize motivational interviewing and collaborate with multidisciplinary teams to ensure whole-person health support.
Job Description
Case managers serve as customer advocates to coordinate, support, and guide care for customers, families, and caregivers to assist with navigating through the healthcare journey.
Responsibilities
- Collaborates with customers in the creation of care plans and documents them in the medical management system.
- Partners to establish goals, interventions, and whole-person health support.
- Interfaces with customers, caregivers, providers, and internal partners via phone, email, text, and chat.
- Utilizes motivational interviewing, behavior change, and shared decision-making.
- Tracks daily activities, follows standard operating procedures, and toggles between multiple systems.
Requirements
- Active and unencumbered Registered Nurse (RN) license in a US state or territory.
- Minimum of two years of full-time direct patient care as an RN.
- Reliable broadband internet connection (at least 10Mbps download / 5Mbps upload) for remote work.
