About The Role
We are looking for dynamic Nurse Practitioners to revolutionize healthcare delivery by conducting virtual wellness visits. Services include adult and pediatric wellness visits and health assessments.
A Day in the Life
- Review caseload and prepare technology tools
- Navigate schedule using modern technology and partner with in-home team members
- Connect with diverse patients to collect information and perform services
- Conduct health risk assessments, medication reviews, and cognitive screenings
- Evaluate vital signs, arrange blood draws, and coordinate comprehensive care
Skills and Requirements
- Board Certified as a Family Nurse Practitioner
- Active FNP license in California
- Credentialed Medicaid provider in California (Medi-Cal)
- Professional, accountable, and compassionate
- Technologically savvy for charting and HIPAA-secure messaging
- Strong communication skills and ability to work independently
