Virtual Clinician Home Evaluations - Nurse Practitioner or Physician Assistant (Full Time) - MN

Role Overview

Signify Health is seeking full-time, board-certified nurse practitioners or physician assistants to conduct Video Visits for Medicare Advantage, Medicaid, and commercial plan members. You will perform approximately 30-minute virtual visits including medical history reviews, patient education, and addressing member questions.

Key Responsibilities

• Conduct online Video Visits with health plan members from a private, secure space.
• Provide 40 hours of weekly availability, consistently providing 320+ capacity units per month.
• Execute a strong visit completion rate, targeting 220+ completed visits per month.
• Maintain training, affiliation, and compliance with Video Visit requirements.
• Engage with regular manager, team, and clinical meetings.
• Meet operational, clinical, and member experience key performance indicators (KPIs).

Requirements & Qualifications

• Maintain active state licenses in coverage areas (FNP or AGNP certifications preferred).
• Maintain active board certification.
• Ability to obtain and maintain multi-state licenses as required.
• Minimum requirements for remote visit delivery: broadband connectivity, video/audio enabled, and a quiet setting with a neutral background.
• Proficiency with information management and technology.
