In-Home Traveler Nurse Practitioner or Physician Assistant Full Time

Signify Health is seeking a full-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their homes and helping bridge gaps in care. Travel is up to 100% of the time across a defined coverage area.

Job Highlights

• Conduct visits lasting about 45 minutes, including medication reviews, medical history reviews, physical evaluations, and diagnostic tests.
• Use an iPad and intuitive clinical workflow without prescribing medicine, ordering lab tests, or altering current treatment regimens.
• Provide diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.
• Be flexible to travel locally, within licensed states, and conduct virtual visits as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification.
• Ability to obtain and maintain multi-state licenses as required.
