Signify Health, part of CVS Health, is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engage with patients in their homes, and bridge gaps in care.
Job Highlights
- Visit Details: 45-minute visits to connect one-on-one, answer health questions, review medication and medical history, and perform a physical evaluation.
- Diagnostic Tests: Perform additional diagnostic tests (e.g., diabetic eye exams, spirometry, bone density screening) if ordered by the health plan.
- Workflow: Use an iPad and intuitive clinical workflow; no requirement to prescribe medicine, order lab tests, or alter treatment regimens.
- Travel: Flexibility to travel locally, within licensed states, and conduct virtual visits as needed.
Work Environment
- Primary workplace is the private residence of health plan members.
- Environments are diverse and may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.
Qualifications
- Active, unrestricted license(s) in coverage area(s).
- Board certification.
- Multi-state licenses preferred.
