Signify Health, part of CVS Health, is seeking a full-time In-Home Health Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations and help bridge gaps in care.

Job Highlights

• Conduct 45-minute in-home health evaluations, including medical history and medication reviews, physical evaluations, and diagnostic tests (such as diabetic eye exams and spirometry).
• Use an iPad and intuitive clinical workflow; no requirement to prescribe medicine, order lab tests, or alter treatment regimens.
• Travel locally and within licensed states for home visits, with virtual visits as needed.

Requirements

• Active, unrestricted medical license in the coverage area.
• Board certification required; multi-state licenses preferred.
• Ability to work in diverse home environments including pets and allergens.
