Care Manager, Registered Nurse (Remote)

Sharecare is looking for a dedicated Care Manager to support the implementation of the Value-Based Care Management program.

Schedule & Compensation

• Schedule: Monday – Friday, 8am – 5pm ET
• Start Date: January 19, 2026
• Salary: $85,000 annually, plus comprehensive benefits package

Essential Job Functions

• Engage telephonically with members, caregivers, and providers to develop comprehensive care plans.
• Collaborate with PCPs, specialists, and other clinicians to meet healthcare goals.
• Assess ongoing care needs and adjust care plans based on changes in condition or transitions in care.
• Coordinate patient education, identify relevant benefit and community resources, and evaluate Social Determinants of Health.
• Perform medication reconciliation at the onset of care plans, after health status changes, and every thirty days.
• Facilitate and monitor care transitions and coordinate post-discharge follow-up.
• Maintain quality documentation and meet annual productivity and quality metrics.

Qualifications

• Current multi-state compact Registered Nurse licensure in state of residence with ability to obtain additional licenses without restriction.
• Minimum 3-5 years of varied clinical experience with telephonic case management experience strongly preferred.
• Strong motivational interviewing, organizational, and problem-solving skills.
• Proficiency in electronic medical records and standard office software.
• Home office setup that satisfies HIPAA requirements and minimum internet connectivity.
