Role Overview

The Care Manager supports the implementation of the Value Based Care Management program by providing high-quality telephonic Case or Care Management. You will partner with members, caregivers, providers, and interdisciplinary care teams to ensure effective care plans and improved health outcomes.

Schedule & Compensation

• Schedule: Monday – Friday, 8:00 AM – 5:00 ET
• Training: Full attendance required during the first six weeks for structured onboarding.
• Salary: $85,000 annually plus comprehensive benefits.

Essential Job Functions

• Engage telephonically with members, caregivers, and providers to develop comprehensive care plans.
• Serve as an extension of the care team by collaborating with primary care physicians, specialists, and clinicians.
• Assess ongoing care needs, coordinate patient education, and evaluate social determinants of health.
• Perform medication reconciliation at the onset of care, after status changes, and every thirty days.
• Facilitate and monitor transitions of care and coordinate post-discharge follow-up.
• Maintain quality documentation and meet productivity and quality metrics.

Qualifications

• Current multi-state compact Registered Nurse licensure in state of residence with ability to obtain additional licenses.
• Minimum 3-5 years of varied clinical experience; telephonic case management experience strongly preferred.
• BSN preferred.
• Proficiency with electronic medical records and Microsoft Office applications.
• Home office meeting all HIPAA and minimum internet connectivity requirements.
