About The Role
This role offers an opportunity to lead a new telehealth-based clinical team focused on managing complex Medicaid populations in Arizona. You will work remotely with an interdisciplinary team to improve patient outcomes.
Requirements
- 1+ years of experience as a licensed Nurse Practitioner
- Active/clear Nurse Practitioner license to practice in the State of Arizona
- Active/Clear DEA
- 1+ years of experience working in a rehab setting / rehabilitation facility centers
- Strong knowledge in HIPAA compliance as it pertains to telehealth
Key Responsibilities
- Serve as the operational lead for the interdisciplinary care team.
- Lead care coordination efforts for complex Medicaid populations.
- Practice autonomously with prescribing authority.
- Manage care delivery primarily through telehealth and remote coordination.
- Provide 24/7 clinical access to help patients avoid unnecessary Emergency Room visits.
- Support patients during hospitalizations and post-discharge transitions.
