Nurse Practitioner/Physician Assistant, Virtual Clinical Support

Role Overview

Part-Time, 24-hour, Remote Nurse Practitioner or Physician Assistant to join our Virtual Clinical Support team. Schedule: 8:30 AM–5:30 PM on Tuesdays, Thursdays, and Fridays.

Key Responsibilities

Clinical In Basket Management

• Prescription Refills: Handle routine prescription requests, review patient history, and ensure refills are appropriate.
• Patient Messages: Respond to patient inquiries through the EHR.
• Out of Office Coverage: Review incoming lab results, flag findings, and initiate follow-up actions.

Virtual Care

• Same Day/Next Visit: Provide virtual consultations for non-urgent issues and escalate cases to the PCP as needed.
• Chronic Disease Management: Manage chronic conditions through virtual visits, ensure care plan adherence, and support PCPs.

Care Coordination

• Referral Management: Manage referrals to specialists or other services.
• Communication: Act as a liaison between the patient, PCP, and other specialists.
• Patient Education: Provide education on conditions, treatment plans, and self-care strategies.

Quality & Process Improvement

• Participate in process improvement, quality assurance initiatives, and ongoing learning regarding telehealth trends and EPIC EHR functionalities.
