About the Role
Our client is seeking a dedicated Telehealth Registered Nurse (RN) with a specialization in Case Management for a fully remote position. You will coordinate services, advocate for patients, and ensure continuity of care from a distance.
Key Responsibilities
- Conduct comprehensive patient assessments via telehealth
- Develop, implement, and monitor individualized care plans
- Coordinate patient care across various settings
- Advocate for patients' needs and ensure timely services
- Educate patients and families about conditions and resources
- Facilitate communication between stakeholders
- Monitor patient progress and maintain accurate EHR documentation
Requirements
- Associate's Degree in Nursing (ADN) required
- Current and unrestricted RN license in the US
- Minimum 3 years of nursing experience (at least 2 in case management)
- Experience with telehealth platforms and EHR systems
