Telehealth Nurse Practitioner, Physician Assistant – Delaware License

Job Description

• Determine individualized treatment plans for patients and facilitate changes when necessary, either independently or in consultation with more experienced providers.
• Use motivational interviewing, behavioral activation, and other behavioral interventions to encourage treatment adherence and conduct behavioral health and psychiatric evaluations for conditions such as depression, anxiety, and insomnia.
• Systematically track treatment response, monitor clinical symptoms, side effects, and adherence, and prescribe or queue appropriate medications.
• Provide patient education on opioid use disorder (OUD) and medications for opioid use disorder (MOUD).
• Document all patient encounters in Ophelia's EHR in a timely and high-quality manner.

Requirements

• Board Certified NP or PA.
• Active DEA license (schedule 3N, 4, 5) or willingness to obtain one upon receiving a job offer.
• Fully licensed to practice in Delaware (additional licenses in NJ, NY, VA, or MD are a plus).
• Ability to provide patient-centered, non-judgmental, and compassionate care to vulnerable or marginalized populations.
• Proficiency in a remote telehealth environment and comfort working across multiple tech platforms.
• Working knowledge of differential diagnosis for common substance use disorders and basic psychopharmacology for opioid use disorder.
• Effective communication and collaboration skills for a team-based care approach.
