Nurse Practitioner/Physician Assistant- Remote Weekend On-Call & Telemedicine

Position Details

• Role: Nurse Practitioner or Physician Assistant
• Location: Remote (Work From Home)
• Schedule: Saturdays & Sundays, 8:00 AM – 5:00 PM
• Type: Part-Time / Weekend Coverage

Responsibilities

• Provide on-call clinical support to nursing staff in Assisted Living, Long Term Care, and Skilled Nursing communities
• Conduct telemedicine evaluations when clinically appropriate
• Assess acute changes in condition and provide treatment recommendations
• Review laboratory and diagnostic results as needed
• Collaborate with facility nursing staff and interdisciplinary care teams
• Assist with timely interventions to help prevent unnecessary emergency department visits and hospitalizations
• Document telemedicine encounters and clinical communications in the Electronic Medical Record
• Support continuity of care and communication with primary providers

Qualifications

• Active Virginia Nurse Practitioner or Physician Assistant license
• DEA license
• Experience in geriatrics, long-term care, primary care, internal medicine, emergency medicine, or hospital medicine
• Strong clinical assessment and decision-making skills
• Excellent communication and collaboration abilities
• Comfortable providing both telemedicine services and telephone triage support
• Experience with Electronic Medical Records
