Key Responsibilities

• Provide primary care to long-term care residents in skilled nursing facilities
• Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment
• Conduct advanced care planning and manage chronic disease conditions
• Prescribe medications including controlled substances in accordance with OH law
• Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

• Current, unrestricted Nurse Practitioner license in OH
• National board certification (AANP or ANCC)
• Active DEA licensure
• Clinical background in adult, family, or geriatric specialties
• Experience in long-term care or skilled nursing facility setting preferred
• Proficiency with electronic medical records (EMR)
