Role Overview
Conduct Medicare Annual Wellness Visits (AWVs) via telehealth for patients located in New York State. This is a 100% remote role focused exclusively on preventive care.
Responsibilities
- Conduct Medicare AWVs via telehealth
- Complete HRA, medication reconciliation, and screening review
- Develop personalized prevention plans
- Document thoroughly and accurately
- Identify care gaps
Requirements
- Active, unrestricted NY NP license
- Eligible to participate in federal healthcare programs
- Comfortable using EHR systems and telehealth platforms
- Strong documentation skills
Preferred
- Medicare or AWV experience
- Multi-state licensure
- Telehealth experience
