Overview
Matrix Medical Network provides comprehensive health assessments to help older adults and at-risk individuals enjoy a better quality of care and identify undiagnosed chronic conditions.
Responsibilities
- Conduct adult and geriatric assessments including medical history, diagnosis, treatment, and health education.
- Administer and order point-of-care and lab-based testing.
- Collaborate with primary care physicians, case managers, social workers, and family members.
- Provide services at members' places of residence, such as homes or skilled nursing facilities.

Qualifications
- Master's Degree or commensurate experience.
- Current NP licensure with prescription authority.
- Board certification in Adult, Adult-Gerontology, Family, Emergency, or Acute Care.
- Current BLS, ACLS, or CPR certification.
- Valid state driver's license and automobile insurance coverage.
- Strong computer skills and communication abilities.
