Position Summary
We are hiring a Pennsylvania-licensed Advanced Practice Provider to deliver high-quality telehealth obesity care, conduct virtual visits, and collaborate with an interdisciplinary team.
Key Responsibilities
- Telehealth Visits: Conduct real-time video visits and follow-up consultations including chronic care management and remote patient monitoring.
- Care Coordination: Collaborate with physicians, nurses, dietitians, and behavioral health specialists to develop individualized care plans.
- Patient Education: Provide health counseling to help patients navigate treatment options.
- Clinical Documentation: Maintain accurate electronic health records.
- Prescription Management: Evaluate patient needs and prescribe appropriate medications.
Qualifications
- Active PA Nurse Practitioner or Physician Assistant license.
- Master's or Doctoral degree from an accredited NP program.
- Minimum of 1 year of clinical experience practicing independently.
- Comfort with telehealth platforms and EHRs.
