About the Role
Lucet is hiring a fully remote Advanced Practice Telehealth Provider (Nurse Practitioner or Physician Assistant) to deliver integrated behavioral and physical health solutions.
What You Will Do
- Deliver comprehensive virtual care including patient assessments, condition management, and medication review.
- Collaborate with multidisciplinary teams and maintain accurate documentation.
- Educate patients on preventive care, disease management, and healthy lifestyle behaviors.
- Adhere to clinical guidelines, regulatory standards, and HIPAA privacy policies.
Required Qualifications
- Active, unrestricted current NY licensure as an NP or PA.
- Minimum of 3600 hours of practice experience or 3 years of clinical experience in primary care or longitudinal care.
- Board Certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner.
- Active Drug Enforcement Administration (DEA) license.
- Current Basic Life Support (BLS) Certification.
- Proficiency in EMR systems.
