Lucet is seeking a remote Advanced Practice Telehealth Provider with an active New York license to deliver comprehensive virtual primary care.
Job Description
Essential Functions
- Clinical Care Delivery & Care Coordination: Deliver comprehensive virtual care, including patient assessments, condition management, medication review, and appropriate escalation while ensuring compliance with telehealth regulations.
- Patient Engagement & Education: Educate patients on preventive care, disease management, and healthy lifestyle behaviors to improve outcomes.
- Quality & Compliance: Adhere to clinical guidelines, regulatory standards, and organizational policies while maintaining accurate, timely documentation.
Required Qualifications
- Active, unrestricted current NY licensure as an NP or PA.
- Minimum of 3,600 hours of practice experience or 3 years of clinical experience in a primary care or longitudinal care setting.
- Board Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant.
- Active Drug Enforcement Administration (DEA) license.
- Current Basic Life Support (BLS) Certification.
- Proficiency in Electronic Medical Record (EMR) systems.
