Lead Nurse Practitioner (NP) – Remote Care Model | $130K–$170K | Phoenix, AZ Area

Role Overview

• Primarily remote / telehealth-based care model focused on complex Medicaid populations
• Opportunity to help shape and lead a new, value-based care approach
• Strong interdisciplinary collaboration without traditional clinic constraints
• Leadership opportunities available on the Lead NP track

Responsibilities

• Lead and coordinate care for complex patient populations
• Practice with a high level of autonomy, including prescribing
• Collaborate with physicians, care managers, and health plans
• Support proactive care management and early intervention
• Assist patients through transitions of care from hospital to home
• Provide virtual care and guidance to reduce unnecessary ER visits

Requirements

• Located in Arizona
• Nurse Practitioner license (AZ)
• Master's degree or higher
• 3–5+ years of clinical and leadership experience
• Experience with telehealth or remote patient management
• Comfortable practicing independently

Compensation & Schedule

• Salary range: $130,000 – $170,000 depending on experience and leadership level
• Mix of W-2 and 1099 options available
• Primarily remote, team-based schedule
