We're working with a healthcare team focused on managing complex Medicaid populations through a telehealth-first, coordinated care model, designed to improve outcomes while reducing unnecessary hospital utilization.

The role
• Lead and coordinate care for complex patient populations
• Practice with a high level of autonomy, including prescribing
• Collaborate with physicians, care managers, and health plans
• Support proactive care management and early intervention
• Assist patients through transitions of care (hospital to home)
• Provide virtual care and guidance to reduce unnecessary ER visits
