Virtual Nurse Practitioner

Overview

EmpowerMe Wellness is on a mission to improve the lives of seniors through an integrated healthcare model. We are hiring a Nurse Practitioner to oversee resident care at a Senior Living Community.

Responsibilities

• Perform initial examinations, physical exams, and patient observations.
• Diagnose and treat physical, emotional, cognitive, and social needs.
• Plan and implement individualized medical plans of care.
• Provide education and treatment for chronic health concerns (incontinence, dementia, mobility, depression, constipation, nutritional issues, dehydration, cognitive impairments).
• Provide restorative, preventative, and palliative care.
• Order, administer, and interpret diagnostic tests.
• Monitor effectiveness of pharmacologic and non-pharmacologic interventions.
• Communicate medical plans of care to leadership, patients, and families.
• Complete timely documentation on patient interactions and visits.

Qualifications

• Master's degree from a nationally accredited Nurse Practitioner program.
• Full practice authority for a minimum of 5 years.
• Current professional nursing licensure in the state of operation.
• Authorization to prescribe medication within the scope of the state practice act.
