RN Care Manager - Remote, nationwide - Full-time
Humana is seeking a full-time, remote RN Care Manager to provide telephonic case management and care coordination for members with multiple chronic conditions. The role involves assessing patient health, developing care plans, and collaborating with a multidisciplinary team.
Job Description
The Care Manager, Telephonic Nurse 2 manages physical, environmental, and psychosocial health issues for members through telephonic outreach. This role is part of the Special Needs Program (SNP) team and is a fully remote position.
Duties
- Telephonically assess Medicare, Medicaid, and Group Account members to create actionable care plans.
- Coordinate care for acute situations and discuss transitions of care for safe discharges.
- Guide members and families toward appropriate community and medical resources.
- Collaborate with multidisciplinary teams including social workers, dietitians, and pharmacists.
Required Qualifications
- Active Registered Nurse (RN) license with no disciplinary action.
- Active Compact nursing license residing in the compact state.
- Minimum of an Associate's degree in Nursing.
- Minimum of 3 years of clinical nursing experience.
- Intermediate to advanced computer skills and familiarity with Microsoft Excel.
- Dedicated home office space with hard-wired high-speed internet meeting speed requirements.
Preferred Qualifications
- BSN or MSN degree in nursing.
- Experience in adult chronic conditions care management.
- Certification in Case Management (CCM).
- Bilingual in English and Spanish.
