- Role: Remote RN Care Manager – Geriatric Care Coordination
- Compensation: $40 – $60/hr with bonus opportunity
- Hours: 30 – 40 hrs/wk, flexible schedule within business hours EST
- Type: 1099, Full-time
Responsibilities
- Serve elderly adults with chronic conditions and their families.
- Educate families on navigating Medicare and the healthcare system.
- Coordinate care across providers and advocate for patient needs.
- Step back from hands-on care to manage a dedicated caseload and prevent unnecessary hospitalizations.
Qualifications
- Hold an RN license.
- Background in case management, home health, hospice, geriatrics, or long-term care.
- Tech-savvy and excited by quickly evolving environments.
- Must be located in the United States.
