About the Role
HHC 24/7 is looking for visionary full-time advanced practice providers to help build the next era of primary care. As a remote APP, you will deliver smarter, faster, and more personalized care integrated with advanced AI from K Health.
Responsibilities
- Provide video-based, comprehensive primary care
- Educate patients on appropriate treatments and care plans
- Manage patient follow-ups for chronic care programs
- Prescribe and refill medication as appropriate
- Drive high-quality care with a focus on patient outcomes
- Perform routine physical examinations and formulate medical, educational, and psychosocial plans
- Develop plans for further evaluation and specialty referrals
- Collaborate with care team members and supervising physicians
