Role Overview
Greater Good Health is hiring a contract Telehealth Nurse Practitioner to deliver care to older adults in access-starved communities.
Responsibilities
- Coordinate continuity of care, prevention, and follow-up for patients recently discharged from the ER or hospital.
- Counsel patients and families regarding discharge plans, medications, and warning signs.
- Conduct annual wellness visits via telehealth.
- Document medical histories, symptoms, and medication information.
- Administer various tests and screenings.
- Collaborate with the Primary Care team to ensure holistic patient care.
- Ensure timely completion of patient records and maintain HIPAA compliance.
Experience and Qualifications
- Current California Nurse Practitioner license.
- NP certification from ANCC or AANP.
- Minimum of 2 years of clinical experience in Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care.
- Current DEA licensure / Prescriptive Authority post-hire.
- Active CPR/BLS Certification.
- Current California Driver’s License.
