About the Role
Galileo is looking for a culturally-competent Nurse Practitioner to join our Population Health team, providing phone and video-based care for post-hospital discharge patients.
Responsibilities
- Cover care needs for the Transitions of Care program.
- Conduct telephonic outreach and scheduled video/phone visits for recently hospitalized patients.
- Focus on medication reconciliation, symptom screening, and ordering needed labs, medications, or supplies.
- Build trust, explain the care model, and connect patients to services.
- Coordinate care across hospital, ED, pharmacy, and community settings.
- Collaborate with the nursing team and meet performance targets.
Qualifications
- Certified Nurse Practitioner in Adult, Family, or Geriatrics.
- Active state licensure in PA.
- 5+ years of post-graduate clinical experience in Primary Care or Geriatrics.
- Experience engaging Medicare and Medicaid populations managing complex, comorbid conditions.
- Motivational interviewing skills and comfort with virtual care delivery.
