Field Remote Nurse Practitioner/Physician Assistant (Must be able to conduct home visits in Ventura County)

Alignment Health is committed to serving seniors and the chronically ill through our Care Anywhere (CAW) program. This role provides an additional level of medical and social support in the patient's home to prevent unnecessary hospitalizations and health complications.

Responsibilities

• Conduct in-home assessments, medication reviews, and health screenings 3.5 days a week, and work remotely 1.5 days a week (Monday–Friday schedule).
• Provide patient education matching desired care with best practices.
• Identify diagnoses for care management and active medical management.
• Partner with the Regional Medical Officer and team to develop care plans.
• Coordinate care with PCPs, specialists, and ancillary providers.
• Lead broader clinical teams including nurses, health coaches, social workers, and care coordinators.

Requirements

• Master's degree from an accredited NP or PA program.
• Active Nurse Practitioner or Physician Assistant license.
• Active RN license and furnishing number (for NPs upon hire).
• Active Nurse Practitioner Board Certification or Physician Assistant Certification.
• NPI Number, DEA, and valid BLS.
• Valid Driver License and current automobile insurance.
• Effective problem solving, communication, and organizational skills.

Nice-to-Haves

• One (1) year of prior clinical or home care experience.
• Previous EMR experience.
• Experience in geriatric patient care.
