FlexStaff is seeking a temporary Nurse Practitioner for a 3-6 month remote assignment with a non-profit organization providing long-term healthcare services.
Schedule
- Monday to Friday
- 8:30 AM – 5:00 PM
- 37.5 weekly hours
Requirements
- NP Board Certified
- NYS Nurse Practitioner License
- DEA Registration and NPI
- Minimum of 2 years of Care Planning experience in a CHHA, LHCSA, acute care, med-surg, critical care, nursing home, or diagnostic & treatment clinic
Responsibilities
- Review discipline-specific documentation for quality and address deficiencies with field staff
- Monitor field staff documentation of interventions and conduct coaching sessions
- Coordinate a continuum of care consistent with member healthcare needs and goals
- Arrange, coordinate, and authorize appropriate community-based services in coordination with the IDT
- Collaborate with PCPs, specialty physicians, and IDT members regarding changes in participant conditions
