What You'll Do
- Provide high quality care to palliative patients in NJ, PA, GA, VA, OK, and KS.
- Assess patients' health by performing physical examinations, studying medical histories, and creating care plans.
- Communicate with office staff and clinical operations across internal platforms.
- Collaborate with the care team to develop holistic, interdisciplinary plans of care.
- Lead sensitive conversations regarding goals of care, advance care planning, and the transition to hospice care.
- Order diagnostic testing and prescribe medications and medical interventions.
What You'll Need
- Board Certified with a valid NP or PA certification in NJ, PA, GA, VA, OK, or KS.
- Strong clinical, communication, and problem-solving skills.
- 2-3 years of palliative, chronic care management, or primary care experience with geriatric patients.
