CareBridge is seeking a 100% virtual Nurse Practitioner licensed in Indiana with an active RN Compact license to provide urgent health care and complex care management. This role includes a $5,000 sign-on bonus and a Monday through Friday schedule with rotating on-call duties.

Position Overview
- Role: 100% Virtual Nurse Practitioner
- Sign-on Bonus: $5,000
- Schedule: Monday – Friday, 8:00 am to 5:00 pm EST and rotating on-call
Responsibilities
- Provide urgent health care via telephone and televideo modalities to Medicaid and dual-eligible members.
- Develop and implement clinical plans of care for adult patients facing chronic and complex conditions.
- Gather history and physical exams, diagnostics, and close gaps in care.
- Educate patients and families regarding medication usage, side effects, illness progression, diet, and nutrition.
- Maintain patient medical records and collaborate with the interdisciplinary team.
- Prescribe medication as permitted by state prescribing authority.
Minimum Requirements
- Master's Degree in Nursing (MSN).
- Active, national NP certification.
- Valid, current, active, and unrestricted Family or Adult Nurse Practitioner license in Indiana.
- Valid, current, active RN Compact license.
- Minimum 2 years of experience in managing complex care cases.
- Experience working with Electronic Medical Records (EMR).
Preferred Qualifications
- DEA registration or eligibility.
- Active Medicaid number in Indiana.
- Experience managing complex care for developmental disabilities and chronically ill patients.
