Job Description
Alignment Health is seeking an Advanced Practice Clinician for the Care Anywhere (CAW) program to provide medical and social support to seniors and chronically ill patients in the comfort of their homes.
Responsibilities
- Conduct in-home assessments 3.5 days a week and work remotely 1.5 days a week
- Perform medication reviews and health screenings
- Provide patient education and identify diagnoses for care management
- Partner with the Regional Medical Officer to develop appropriate care plans
- Coordinate care with PCPs, specialists, and ancillary providers
- Lead a broader clinical team including nurses, health coaches, social workers, and care coordinators
Requirements
- Master’s degree from an accredited NP or PA program
- Active Nurse Practitioner or Physician Assistant license and board certification
- Active DEA, NPI, BLS, valid driver's license, and auto insurance
- Strong communication, problem-solving, and organizational skills
