Remote Nurse Practitioner/ Physician Assistant - Virtual Care (Must have CA License)

Alignment Health is seeking a remote Nurse Practitioner or Physician Assistant to join the Virtual Care Center (VCC) team, providing 24/7 telehealth services to senior patients.

Responsibilities

• Conduct virtual visits and comprehensive assessments, including past medical history, review of symptoms, physical examinations, medication reviews, and cognitive/depression screenings.
• Provide patient education on chronic conditions, self-care, sick-day plans, medication compliance, and community resources.
• Formulate lists of current and past medical conditions and identify diagnoses for care and active medical management.
• Communicate assessment findings to generate post-visit summary notes for PCPs and identify gaps in care.
• Participate in weekly interdisciplinary team meetings to develop appropriate care plans.
• Ensure compliance with all HIPAA regulations and protect health information.

Requirements

• Experience: At least 1 year of prior clinical experience (3 years preferred; geriatric care experience preferred).
• Education: Master’s degree from an accredited NP or PA program.
• Licensure: Active Nurse Practitioner with Active RN Licensure or Physician Assistant Board Licensure.
• Certifications: Valid BLS upon start; NPI number upon start; DEA number and NCCPA board certification within 6 months of start.
