Job Description
Alignment Health is seeking a Field Remote Nurse Practitioner/Physician Assistant for the Care Anywhere (CAW) program to provide medical and social support to seniors in the comfort of their homes.
Responsibilities
- Conduct in-home assessments, medication reviews, and health screenings 3.5 days a week; work remotely 1.5 days a week (Monday through Friday schedule).
- Provide patient education by matching desired care with best clinical practices.
- Identify diagnoses for care management and active medical management.
- Partner with Regional Medical Officers and multidisciplinary team members to develop care plans.
- Coordinate care with PCPs, specialists, and ancillary providers.
- Lead broader clinical teams including nurses, health coaches, social workers, and care coordinators.
Job Requirements
- Education: Master's degree from an accredited NP or PA program.
- Experience: None required; 1 year of prior clinical or home care experience preferred.
- Skills: Positive communication, problem-solving, organization, and time management skills in a fast-paced environment.
