Key Responsibilities

• Provide telephonic and virtual patient assessments, care coordination, and ongoing monitoring for assigned patient populations.
• Deliver patient and family education related to conditions, treatment plans, medications, and self-management strategies.
• Develop, implement, and update individualized care plans based on clinical guidelines and patient needs.
• Monitor patient progress, escalate clinical concerns appropriately, and support early identification of deterioration or barriers to care.
• Coordinate services across providers, payers, and community resources to ensure continuity of care and adherence to care plans.
• Conduct outreach to close care gaps, schedule appointments, manage transitions of care, and support medication reconciliation.
• Document all interactions accurately and promptly in electronic health records (EHR) and care management platforms.
• Collaborate with clinical and operational team members to optimize workflows and patient engagement.
• Support quality improvement initiatives, performance metrics, and program-level reporting.
