About the Role
As a Devoted Preventative Primary Care Provider, you will be part of an interdisciplinary clinical team focused on improving member health and dignity. This role involves conducting 1-hour comprehensive clinical assessments, supporting chronic condition management, and emphasizing preventative and proactive care via telehealth.
What You’ll Do
- Conduct primarily telehealth video visits with limited occasional in-home visits.
- Perform comprehensive assessments, chronic condition support, and medication optimization.
- Identify care gaps and contribute to individualized care plans.
- Utilize integrated technology and AI-enabled workflows.
Required Qualifications
- Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license.
- 3+ years of outpatient clinical practice (primary care, family medicine, internal medicine, or geriatrics).
- Active BLS certification.
- Comfort with telehealth and electronic documentation systems.
