Devoted Health is hiring a Virtual Primary Care Nurse Practitioner to provide comprehensive telehealth care, manage chronic and acute conditions, and coordinate multidisciplinary treatment.
Job Description
Provide virtual primary care through an innovative telehealth platform. Deliver comprehensive care and foster strong patient relationships.
Responsibilities
- Provide virtual primary care visits via telehealth, including assessments, chronic disease management, and acute care.
- Diagnose and manage chronic conditions like hypertension, diabetes, COPD, and heart disease.
- Evaluate and treat acute concerns such as infections and respiratory illnesses.
- Order and interpret lab tests, imaging, and diagnostics.
- Manage medication prescriptions and refills.
- Provide health coaching, lifestyle guidance, and patient education.
- Coordinate referrals and home-based services.
- Participate in clinical case reviews and team huddles.
Qualifications
- Active Nurse Practitioner (APRN) or Physician Assistant license.
- Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
- Minimum 2 years of primary care, urgent care, or internal medicine experience.
- Active BLS certification.
