Nurse Practitioner - Maryland Remote

Role Overview

Join DaVita Integrated Kidney Care (IKC) as a remote Nurse Practitioner in Maryland. Help patients with complex kidney conditions navigate the healthcare system and receive holistic, integrated care.

Position Details

• Location: Remote in Maryland (work from home).
• Primary Responsibilities:
  • Complete Comprehensive Health Evaluations (CHEs) to assess and manage comorbid conditions.
  • Manage Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD) patients to slow disease progression and reduce hospitalizations.
  • Utilize prescriptive authority, medication management, and diagnostic interpretation.
  • Lead the clinical team to ensure care coordination aligns with medical treatment plans.
• Schedule: Monday-Friday, full-time, self-managed schedule. No nights, weekends, or on-call.

Requirements

• Minimum 2 years of experience as a Nurse Practitioner.
• Current, unrestricted NP license in Maryland and DEA eligibility within 90 days.
• Current CPR/BLS certification.
• Valid driver’s license and insurance.
• Intermediate computer skills.
