Nurse Practitioner - Remote
DaVita is seeking a remote Nurse Practitioner to manage complex chronic kidney disease and end-stage renal disease patients in Georgia. The role focuses on comprehensive health evaluations, care management, and reducing hospitalizations with a flexible, self-managed schedule.

Position Details
- Location: Remote covering specific areas in Georgia
- Responsibilities: Complete Comprehensive Health Evaluations (CHEs), manage CKD and ESKD patients, utilize prescriptive authority, and lead clinical care coordination.
- Schedule: Monday-Friday, full-time, self-managed. No nights, weekends, or on-call.
Requirements
- Minimum 2 years of experience as a Nurse Practitioner
- Current, unrestricted NP license and DEA license (or eligibility within 90 days)
- Current CPR/BLS certification
- Valid driver's license and insurance
Preferred Qualifications
- Experience in Value-Based Care or Population Health
- 5+ years of experience as an FNP, Primary Care NP, or in Internal Medicine, Cardiology, Nephrology, or Endocrinology
- EHR experience (e.g., Cerner)
