In-Home Traveler Nurse Practitioner or Physician Assistant Full Time - MD

Role Overview

Signify Health is seeking a Full-Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes.

Key Responsibilities

• Conduct in-home health evaluations lasting about 45 minutes.
• Perform medication and medical history reviews, physical evaluations, and additional diagnostic tests if ordered.
• Use an iPad and intuitive clinical workflow for each evaluation.
• Travel up to 100% of the time across a defined coverage area, including potential overnight stays, car rentals, and flights.
• Conduct virtual visits as needed.

Qualifications

• Active, unrestricted license(s) in the coverage area.
• Board certification.
• Ability to obtain and maintain multi-state licenses as required.
