Role Overview

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. Engage with people in the comfort of their homes, build trusted relationships, and help bridge gaps in care.

Job Highlights

• Conduct 45-minute in-home health visits.
• Medication and medical history review, physical evaluation, and additional diagnostic tests if ordered by the health plan (e.g., diabetic eye exams, spirometry).
• Use an iPad and intuitive clinical workflow.
• No requirement to prescribe medicine, order lab tests, or alter current treatment regimens.

Responsibilities

• Bring your heart into every visit and improve health outcomes.
• Visit people nationwide to connect them to the right care.
• Fulfill the requirements of the specified clinician role.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services products.

Work Environment

• Primary workplace is the private residence of health plan members.
• Environments are diverse and may contain household pets, secondhand smoke, and other potential allergens or irritants.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
